We ask for the safety of our group that all participants have the COVID-19 Vaccination OR receive a negative COVID-19 test 72 hours prior to engaging in the workshop. Please check one of the boxes below to acknowledge which option you will adhere to:*

I have completed, or will complete, the COVID-19 vaccination prior to the start of the workshop
I will take a COVID-19 test within 72 hours of the start of the workshop and will attend only if the results are negative

Acknowledgment and Consent*

In light of the recent CDC updates in mandates and regulations, I acknowledge there is still a risk of becoming exposed to and/or infected by Coronavirus/COVID-19. I acknowledge I voluntarily seek services provided by Productive Learning LLC and know Productive Learning LLC cannot guarantee I will not become infected with the Coronavirus/COVID-19. I hereby release and agree to hold Productive Learning LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, costs, expenses, etc., that result from, or have relation to, Coronavirus/COVID-19, or that may otherwise arise in any way in connection with any services received from Productive Learning LLC. This liability waiver and release extends to Productive Learning together with all owners, partners, and employees.
